Terms and Conditions – This form must be read and signed by all clients of My Vacation Lady and Star Enterprises I Ltd Travel.

Credit Card Authorization – A credit card authorization form will be required for credit card payments (both deposits and final payments). By filling out this form you are agreeing to pay the amount charged by the travel supplier for you vacation.

Insurance Waiver – This form will be required when you have been offered travel insurance but are declining it. Travel Insurance is strongly recommended for all vacation plans.

Parental Consent Form – If you are taking children with you on your vacation and you are not their parents or legal guardians, you must have this form filled out and signed by their parents. This form must also be notarized.
